EPC / Medicare Chronic Disease Management (CDM)

Power Potential provides evidence-based physiotherapy and exercise physiology services under the Medicare Chronic Disease Management (CDM) program, supporting individuals living with long-term or complex health conditions.

If you have a chronic condition that has lasted, or is expected to last, six months or more, your GP may refer you for allied health treatment through a CDM plan. Our team works closely with you and your GP to improve function, manage symptoms and support long-term health outcomes.

Treatment focuses on practical, sustainable strategies to help you move better, reduce pain and maintain independence in daily life.

What to Expect During Your Session

  • Comprehensive initial assessment

    • Progress updates provided to your referring GP

Is CDM Right for You?

You may be eligible for CDM services if you:

  • Have a chronic condition lasting (or expected to last) six months or more
    • Have a GP Management Plan and Team Care Arrangement
    • Need support to manage pain, mobility, strength or function
    • Want structured guidance to improve long-term health and independence

Medicare provides rebates for a limited number of allied health sessions per calendar year under a valid GP referral.
